- Your NI number
- ID Photo
- Passport/Visa Number
- Copy/picture of Passport data page
- Proof of address document
- 2 Employment References
- Proof of education – Certificate copies
- Current DBS number if you have one

Please take time to check whether you are ready to apply for registration. Ensure you have what is listed in the checklist above. Some of the items on the list are required, the application form will not allow you to proceed if you have not provided them.
